Yes, it's almost in realtime. Photoshop says that its document was changed only when the user releases the mouse. So the process of the drawing cannot be captured in realtime, but as soon as the document was changed - the image appears in vvvv.
And yes, vvvv will send textures to Photoshop, whenever you want.
vvvv can send its texture to any location inside PSD (incl. groups and smart objects). It takes a second, because at first the texture appears in Photoshop in a separate Document (this is the only option Adobe offers) and then it is copied into any location inside of any document by the script (also coming from vvvv).
The connection is encrypted, so it was Tebjan who knew about the PKCS7 and the likes.

At the moment (the most simple case) the whole image is sent to vvvv.
